About me card
A small card to show staff how to help you, or to use if someone gets separated.
Fill in the form and a wallet-size card (85 × 54 mm) builds as you type. Print it, save it, share it or email it to yourself. Leave out anything you do not want on the card.
Nothing is stored or sent by us. Everything you type stays on this device and is not saved when you close the page. A printed card, a downloaded file or an email contains whatever you typed, so think about where you keep it. The email is sent from your own email app, not through our site. A first name or nickname is enough; please avoid full names, addresses and medical record numbers if you do not need them.

A saved file or an email contains whatever you typed. Keep it somewhere you are happy with, and delete it when you no longer need it.
"Email this to myself" opens your own email app with the card text filled in. You type your own address and send it from your device. It does not pass through our site.
